Most of the Year 13 students scored zero for their internal assessments which could leave them short of the required marks to be able to enter the National University of Samoa.
Parents have expressed their frustration as a similar problem occurred last year.
Afamasaga Dr Karoline Fuata'i of the Ministry of Education, Sports and Culture has blamed it on human and computer error.
